Instructions for Completing the Application Form:

The form is short, simple, and self-explanatory.  Be sure that you have the project well planned and budgeted before completing the form.  The person submitting the proposal may or may not be the same as the person managing the funds.  Make sure the “Proposal description” includes why the project is needed.  Make sure that the budget is itemized and as specific as possible.  List the approximate start and end dates of the project; if you will need time beyond one year to expend funds, please explain.  Allow time to secure Program Council approval.  The Program Council meets on April  1 and will need to have information about your proposal ahead of time (not necessarily the completed proposal).  Contact Sue Dudley at scdudley35@bellsouth.net to arrange for Program Council approval.

Funding Restrictions:  Grants will not be made for current operating expenses of the UUFG, such as salaries, utilities, and debt service.  One-time projects that have no recurring costs or that can be continued from other funds are encouraged.

Reporting Requirements:  Quarterly reports are due until the project is completed (see attached form).

Method of Payment:  Upon grant approval, payment will be made to an off-budget account of the Fellowship, and UUFG budgetary policies and guidelines will apply for fund expenditures.
